Hi Abraxas,
for HTML help, I use PSPad as editor to write HTML files and then "Html Help Workshop" (http://www.microsoft.com/downloads/details.aspx?familyid=00535334-c8a6-452f-9aa0-d597d16580cc&displaylang=en) from Microsoft to compile it to CHM.
Advantages: Free
Disadvantages: HTML Help Workshop is older app, with very basic functionality.

I use HelpNDoc.

It's free for freeware and very cheap for commercial needs.
